Another new guy... - TnG80 - 03-12-2025

Short history... Had sleep apnea for years (15ish?) quit smoking last April. Gained 60 plus lbs putting me over 305lbs. End of december i decided to get my sh*t together. Got a sleep study, the doc said I have "world class" apnea... AHI of over 50. Started on my machine. Immediate improvement in quality of life/sleep.  

I've lost 40ish lbs so far. according to my oscar data, my OA is nearly non existent. My AHI varies from 5.5 to 20's. Average of 15. Most events are CA. Pressures are 16 and 25. PS is 4. I use an F20 and F40 mask. I was sleeping a solid 8 hours from day one, but my sleep has reduced to about 6 hours. I feel fine and have enough energy to work 10 hour days for 2 weeks at a time. 

The CA's bother me. 6 hours is fine, but i think i could use another hour. Should I start messing with settings? Ask the doc to review my data? Ask for another sleep study? I find it hard to complain because i feel so much better, but i think think things can be improved.Oh, and i have mad aerophagia... the mornings are filled with farts and burp. According to my wife i fart at night as well...

With that many CAs, you need a completely different therapy than what you are receiving.  Have you ever had a polysomnography...an overnight sleep lab?  The results, if verified, would have the RT doing the test report your full results and you would be referred to a sleep specialist of some kind.  That person, if competent, would urge you to consider using an adaptive servo-ventilator because that kind of PAP therapy is about all that can deal with CAs.

The original diagnosis was from an at home and a overnight study. I need to check my records, but I don't believe I was having CAs during my overnight.

There are settings on the AirCurve 10 VAuto to address the CA.  Do you have a current Clinicians Manual? Get one here:   https://www.apneaboard.com/adjust-cpap-p...tup-manual

There is a setting for "Response" in the VAuto, that if you increase it, could possibly reduce your CA events. The Clinician's manual will help navigate the menu, to enable you to set Response.

We really need to see some OSCAR Reports in order to fully assist you.

It would help us help you if you posted a redacted copy of your sleep study.

It looks like you have already downloaded Oscar. Post a couple of charts and we'll be able to offer advice right now.

Congratulations on turning your life around, quitting smoking and the weight loss, those are not easy, and both respectable.

If you will not be home for a while, I hope you have an opportunity to review some of our WIKI articles. That will give you a leg up in working with the forum members.

In my signature below, you will find some links to various sections of the WIKI. The more familiar you are with the phraseology and terminology associated with OSCAR, the better our communications will be.

For our forum members to better assist you, the entire left panel will need to be visible, it may take a couple of screenshots to get the different sections of the left hand panel in full view, that is fine as long as we can see the information. If you grab the right border of the left hand panel and drag it to the right just a little, the information in the Statistics area will be displayed in single rows, allowing more information to be displayed. The information in the Machine Settings area and Session Information will be important for your next posting. There again see OSCAR Chart Organization in my signature. 

There are some Timing, trigger and cycle settings that may provide some relief from the CA,  we need to see your machine settings and more OSCAR Reports for that. I doubt if two week will be sufficient time to send you home fart free, but you can assure your wife, we are cognizant of the effect of aerophagia, and working towards her relief, and your improved therapy. 

There are some forum members who are more familiar with spontaneous Bi-Level PAP therapy, who I will invite to look in on your therapy thread. We will need to see the Machine Settings,

A few more questions, age (roughly), height and general health. I noticed you had some SpO2 desaturations during your sleep lab, do you wear a Pulse/Oximeter during sleep now?
