[CPAP] I want my Dreamstation back
#31
RE: I want my Dreamstation back
I've forgotten, what EPR settings have you tried? If it's full time 3, then that's as high as that goes. If you feel you need to attack the flow limits any further, this means increasing the Min pressure.

You need to decide if that's needed. It comes down to your comfort. CA and OA are slight, and zero isn't the goal anyway. Flow limits are Max of .27, so there's some there to work on if you think this needs improved. One thing to note is ResMed pressures are edited at steps of .2 not .5 like Respironics. So you could increase Min by .2 if you choose to try it. Now realistically any edit is to improve comfort. You could add .2 and see if there's an improvement but you don't need to keep it if it didn't help. You could then decide to go up again to 9.4 or go back to Min 9.0.

Thanks. Yes. It is already at 3 full time. I am glad though that you elaborated on increasing the minimum pressure in very small increments. I can certainly try that. I just wonder if the whole flow limits situation might explain why I’ve never felt all that much more rested all these years. Appreciate your input.

You're welcome. As I said feel free to edit to see if it helps. If it doesn't you can always change it back.

Is this significant to you? It sounds like it.  Documenting this could justify getting a VAuto (BiLevel) which could better treat your flow Limits.

Turn EPR off for 1 full night, document how you feel. Be sure to save your charts.  This will more closely mimic your Dreamstation.

Gideon.... I don't know if it's significant... it's a whole new concept to me.  Looking back at the Oscar reports from the Dreamstation, it appears that flow limits weren't even reported then so I never knew it was something to monitor.  I don't miss the Dreamstation as much as I did when the AirSense was waking me up all the time.  Seems like lowering the max pressure solved that problem for the most part.  And change is always hard!

Sarcastic Dave just said my AHI wouldn't warrant getting a BiLevel... and to be honest, I haven't finished paying for either the Dreamstation or the AirSense so I'm not going to be in a hurry to get yet another machine!

the call on whether or not to get a VAuto is yours, your AHI does appear to be solid. Keeping changes depends on which settings you feel are better. Yes, very subjective.

Minor changes such as slowly raising min pressure to see which works better FOR YOU is a good thing.

If flow limits are really going to be an issue, then yes I can see value in a VAuto. Since your EPR is maxed out, the only way to address them now is how I'd said just a bit ago, increase Min. The choice is always yours to make, we can help either direction.

Staying on AutoSet, again may mean increasing pressure to combat the FL. If that lowers your comfort, you can build a case for VAuto with this in mind. You'll need to collect some symptoms and complaints in a list to tell the doctor why you need it. Impossible? No. A fight? Yes almost always there's a fight to change machines.

I honestly don't know how big an issue the flow limits are... and as for changing machines...  No doctor has ever prescribed a machine for me except for the very first one years ago.  Once I saw what a ripoff getting a machine thru insurance was at the time I just started buying my own and have continued to do so. This forum taught me how to adjust them as needed and how to record the stats.  I'm actually thinking that once I have some more information gathered I will ask my PCP for a referral to a sleep medicine doctor since I've never actually seen one.  Just had PCPs who thought they knew enough about all of this and I'm beginning to realize that that was probably not the case!

I will start by increasing the min and see what happens... thanks again.
