New CPAP User - Help Please - ResMed Airsense 11 Autoset

Question
New CPAP User - Help Please - ResMed Airsense 11 Autoset
Good Morning,

I found this site because I had posted in R about the CPAP AHIs being Unstable. I was unable to get data, since I did not have an SD card in the machine for the first week. I only have complete data for last night.

The AHI for the first week was

N1 - 35
N2 - 17
N3 - 19
N4 - 33
N5 - 26
N6 - 27
N7 - 39
N8 - 7
N9 - 8 (last night)

Mask Seal and Mask on/off were both good according to the app.
It's on a dr set pressure of 8.

The AHI at the sleep study was 15.2

Other information:
USA - 35 YO male - Healthy Weight - OSA caused by his jaw being back far - He's not noticing much of a difference in sleep quality or feeling refreshed when he's awake.

He's a mouth breather, and uses a full face mask. Airfit F20 Size Medium

Any advice on why his AHI is so unstable, and how to make this work better for him would be very appreciated. Thank You.

You appear to be having some positional apneas, the apnea clusters are a sign of this.  You are most likely chin tucking, your neck/throat position is cutting off your air way.  Imagine a garden hose bent over, stopping the flow of water, CPAP pressure will not help with this.   You need to either get a lower pillow or if you sleep on your back try sleeping on your side or try using a soft cervical collar, it helps with some people.    Also, if you do not have to use the ramp, turn it off, you aren't getting any benefit from the CPAP to control apneas when it is running.  Your flow limits are quite high and may be causing arousals, also you pressure may not be quite high enough, but first I think that you should get the positional apnea under control.

Search WIKI on this site for more info on positional apnea.

Thank you!

So for now, just have him sleep on side, instead of on back, and turn off ramp?

Is flow limits something I need to change?
